The Human Resources Department at Logan College of Chiropractic/University Programs has found that many employees have similar inquiries. Listed below are the most commonly asked questions and answers for your convenience.
Q: How soon should I inform HR if I have a life-changing event (i.e. birth, adoption, marriage, divorce, or loss in coverage)?
A: Employees have 31 days from the date of the life-changing event to inform Human Resources. Necessary paperwork must be completed in a timely manner if there are additions/changes in insurance coverage.
Q: What do I do if I have questions or problems regarding the way my medical or dental bills are being covered?
A: For Health Insurance, call BlueCross/BlueShield Client Services, which is on the back of the insurance card (800) 392-1104.
A: For Dental Insurance, call MetLife Customer Service: (800) 275-4038. (Due to HIPPA privacy standards, most information regarding your doctor and dentist visits, or hospital care is considered protected health information. In order for us to help you, we would need to have you sign a form to allow the insurance company to release this information to us. We can arrange to do this if you call one of the above numbers and do not receive a satisfactory resolution to your problem.)
Q: What do I do if I lose my insurance card?
A: Contact the Human Resources Office and we will order you a new insurance card. Once the card is ordered it will be mailed to your home address usually within 10 working days.
Q: When do dependent children lose coverage under the health insurance plan?
A: At the end of the calendar year after they turn age 25.
Q: How do I change my beneficiary on my Life insurance or the Retirement plan?
A: For Life Insurance, stop by the HR office and complete the necessary form.
A: For the TIAA-CREF Retirement Plan, you may go online to www.tiaa-cref.org, or call (800) 842-2252.
Q: At what point am I vested in the Retirement plan?
A: Employees are eligible for Logan’s 5 percent contribution after two years of employment. You are immediately 100 percent vested.
Q: How do I adjust my withholdings on my federal and/or state tax forms?
A: Contact Human Resources to complete the Federal and State forms.
Q: Does vacation roll over or do I lose it if I do not take it?
A: Vacation does not roll over. Some exceptions may be made if you become eligible for your vacation in the last quarter of the year. Extensions of up to three months into the following year may be made with written approvals.
