Financial Aid Check List

The financial aid checklist is designed to provide you with information about the documents the Office of Financial Aid at Logan College of Chiropractic/University Programs requires and a suggested timeline by which to complete the process.

Each application is considered individually, which may require more information to be submitted. Once you have initiated your admissions application, the Office of Financial Aid will notify you on what documents are needed to complete your file.
The information in brackets on the checklist indicates the time prior to your first day of class that each step should be completed by in order to receive optimum consideration of your application.
The Office of Financial Aid can make an initial determination of your eligibility when all items have been received.
Once your file is complete, you will be eligible to receive an award letter. The earliest these are mailed is six to eight weeks prior to your scheduled start date. The award letter details your maximum eligibility for financial aid. Additional forms may accompany your award letter, which must be submitted to secure funds.

When you have been admitted, you will be contacted to submit any incomplete paperwork.

	2012-2013 Free Application for Federal Student Aid (FAFSA): this form must be submitted to the federal processor after it has been completed. Title IV Code: 004703 [5 months]
	Logan College Financial Aid Application: Mail this form to the Financial Aid Office. [4 months]
	Verification Form: The Office of Financial Aid will send you this form if it has been determined your application has been selected for verification by the Department of Education. Forward the completed Verification Form and signed copies of your (and your spouse’s or parents’ if applicable) federal tax forms to the Office of Financial Aid. [2 months]
	Award Letter: You must return a signed copy of the award letter to receive the aid. Should you wish to decline any or all of the aid, please return a copy of the award letter along with a detailed statement outlining which aid you would like to accept and/or decline.
	Stafford Loan Application: You will need to go online and fill out the Master Promissory Note using e-signature. You will be notified of your lender and the applicable website.
	Perkins Loan Application: If you have awarded and wish to receive a Perkins Loan, you must complete the application and forward to the Office of Financial Aid.

**Miscellaneous: if additional forms or requests for information have been mailed to you along with your award letter, you will need to complete them and return to the Office of Financial Aid to secure funds.
