Doctor of Chiropractic Program Application

Welcome!  We are excited that you are interested in attending the doctorate program at Logan College of Chiropractic.  You may submit your application online by clicking on the link at the bottom of this page.  Your application will be processed by the Admissions department as soon as it is received. 

Please be sure to review our admissions requirements and read the information below before completing the application for admission.  If you have any questions regarding this process, contact the Admissions office for more information.

     1.                 The application must be completed in full.

2.                  A non-refundable application fee of $50 (U.S. dollars) must be enclosed with applications.  For your convenience, you may charge the fee to your MasterCard, Visa, American Express, or Discover through our online Secure Area.

3.                  Official transcripts from all colleges and universities attended must be sent directly to the Logan College of Chiropractic Admissions office.

4.                  All applicants need a letter of recommendation sent directly to the Admissions office from a licensed Doctor of Chiropractic.  Contact the Admissions office for a referral if you are unable to provide this reference.

5.                  All applicants must provide the names and addresses of 3 people who, by virtue of their occupation and/or credentials, would be considered judges of your professional potential.

6.                  A personal interview is required.  Under certain circumstances this interview can be held off-campus.  Logan College of Chiropractic provides a complimentary hotel room for out of town prospective students.  Please call to make arrangements.

7.                  Additional requirements for International Students:

a.      International Students may be required to submit transcripts to an outside evaluating agency if transcripts are not transcribed in English or are based on a different education system then that of the United States.

b.      International Students must submit proof of English proficiency, unless they have earned a degree in the United States or English is their first language.  To verify English proficiency, students are required to earn a satisfactory score on either the internet-based TOEFL or the paper-based TOEFL.

c.       International Students, outside of Canadian citizens, will be required to obtain an F-1 visa for study.  All students are recommended to have a valid passport.
